Background

Orthopedic Specialists of Oakland County (OSOC), a leading orthopedic practice based in Bloomfield Hills, Michigan, achieved a national milestone in May 2026: Dr. Safa S. Kassab became the first surgeon in the United States to perform a total knee replacement using Pixee Medical's Knee+ NexSight augmented reality glasses.

Dr. Kassab is the nation's foremost authority on AR-assisted total knee arthroplasty, having performed nearly 2,000 AR-guided knee replacements since 2023 — more than any other surgeon globally using the Pixee platform.

The Deployment

The Knee+ NexSight system is an FDA-cleared wearable heads-up display designed specifically for ambulatory surgery center (ASC) environments, where total knee arthroplasty (TKA) is increasingly being performed. The device projects critical navigational data — including bone alignment, implant positioning, and surgical landmarks — as a virtual screen directly into the surgeon's field of view, eliminating the need to look away from the operative site.

Unlike robotic navigation systems that introduce automated steps, the Knee+ NexSight approach keeps the surgeon in complete control at every stage. The system is compact, requires no dedicated console, and uses no disposables, making it well-suited for high-throughput surgical environments.

Hardware and Software

The Knee+ NexSight smart glasses are built on a new, more compact AR platform compared to Pixee Medical's prior generation. Voice command input is supported. The system delivers what Pixee Medical describes as robotic-level accuracy in implant alignment without the cost or complexity of robotic systems. Pixee Medical's first-generation products have been used in over 10,000 procedures across more than 20 countries.

Outcomes and Context

Dr. Kassab and the OSOC team have invested extensively in surgeon training, patient education, and outcomes tracking alongside their clinical adoption of the platform. OSOC serves patients across Metro Detroit and Southeast Michigan from its Bloomfield Hills location. Dr. Kassab serves as a paid consultant to Pixee Medical.
