Tri-Service General Hospital trains Taiwan's military medics for mass-casualty triage in VR

ATLS scenarios drop 15 or 16 earthquake casualties at once, so students, residents, nurses and EMTs rehearse triage without a live drill.

What was deployed

Tri-Service General Hospital, Taiwan's flagship military medical centre in Taipei, has developed a virtual-reality trauma training system in collaboration with Taiwanese technology company HTC. The system builds Advanced Trauma Life Support (ATLS) protocols into simulated mass-casualty incidents, and the hospital describes it as the first VR-based trauma training system of its kind in Taiwan.

How it is used

The current focus is triage under load. A scenario might place a trainee in an emergency department where an earthquake has delivered 15 or 16 casualties at once, with injuries of varying severity, requiring them to assess and prioritise each patient quickly.

Trainees can repeat a scenario and review their own performance afterwards. Instructors can vary the setting and the injury patterns rather than running participants through one fixed scenario repeatedly.

"VR provides a very realistic immersive environment," said Yang Chih-Jen, an attending physician in the hospital's Department of Emergency Medicine, who added that the technology reduces the manpower, space and cost that conventional mass-casualty drills demand.

Who trains on it

Medical students rotating through the emergency department are the main users today. The system is also available to resident physicians, nurses and emergency medical technicians.

Operational context

The hospital presented the trainer alongside its Forward Surgical Team (FST) programme — small, highly mobile surgical teams that deploy close to the front line to stabilise casualties in battlefield or disaster conditions before they can be evacuated to a major hospital. FST personnel train with the military roughly monthly, drawing trauma surgeons, orthopaedic and cardiovascular specialists, nurses, anaesthesiologists and emergency medical technicians from military hospitals across Taiwan.

Roadmap

Yang said the system could be extended to more austere conditions, such as limited lighting or shortages of medical equipment, forcing trainees to work with constrained resources. Scenarios involving a complete loss of electricity or water supplies have not yet been incorporated.
